2010 National Jamboree Personalized Medical Forms Being Distributed

Last month we informed you about the 2010 National Scout Jamboree personalized health forms being delayed. The personalized health records are now being distributed. Keep in mind, from the original delayed message, that the forms will take several weeks to be delivered to everyone. If you have not recieved your form yet, do not worry, it should be arriving shortly.

When you do recieve your personalized bar coded Medical Record you will recieve two emails. The first, will have the medical form attached. The form is an adobe acrobat form that is password protected. Your name and contact information will be pre-filled. This email also includes the directions:

Please open the attached Jamboree Medical Form using the password sent to you in a separate email. (Viewing the attachment requires Adobe Reader software. If you do not already have it, go to http://get.adobe.com/reader/ and download it for free.)

You will need to complete Part A by entering in the information into the form before printing it. After printing it, take it to your physician, physician assistant or nurse practitioner to have Part B completed by him or her. Once Parts A, B, and C are complete, and you have copied your insurance card onto the last sheet, follow these instructions:

Youth Participant or Adult Leader: Return the form to the Scoutmaster of your Jamboree Troop.

Jamboree Staff: Please fax the form to 972-580-7860.

The second email will contain your password to open the document. This email came within seconds of the first email message with the medical form attached.

    I recently saw an article regarding the distribution of the medical form for Jamboree participants and leaders. As the staff advisor for our 5 jambo troops, I would like to share this information the scouts and leaders.

    Can you tell me the source where you found this information? Did you receive one of the electronic medical forms?

    Just want to be sure I am sharing accurate and factual information before I distribute.

    I received my personal bar-coded medical form. The quoted part above in light blue is the text of the email.

    I thought the new medical forms effective 1 Jan 2010 was suppose to replace ALL medical forms thatthe BSA uses, including Jambo?

    @ Jerry – Supposedly this is just a personalized (ie. Bar coded version) of the new medical forms. This is so they can easily track the mass number of forms and participants. I didn’t look close enough to see if it was the same form or not, I do know the formating is different.

    I have not yet recieved the forms and it is January this thing sounds like more trouble than it’s worth.

    @Ferade – Check out http://www.bsajamboree.org/MedicalProcess.aspx to see how you can request a resend. Don’t just email them the request, first check your information in myscouting as described, just to be on the safe side.
